Job Details:

* Minimum of 1 year of experience required
* Epic charting system experience preferred
* Scheduling type: Other
* Weekend coverage required
* Patient caseload of up to 35 patients
* Patient ratio of 35 on days and weekends
* Serves adult and geriatric patient populations
* Required certification: MSW
* Compact license must be in hand at time of submission
* Skills required include adult and child protective services, advance directives, care coordination, case management, discharge planning, documentation of care delivery and patient teaching, drug and alcohol use and treatment options, emergency department, ICU, medical surgical, suicide/homicide/self-harm assessment, and coordination with internal and external team members
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* All requested time off and scheduling requests must be submitted upfront during submittal
* Travelers may be expected to float between nearby sister facilities; refusal to float may result in termination or forfeiture of guaranteed hours
* 2 professional references required, covering 1 year within the last 3 years, to be submitted during compliance process prior to start

This is a Critical Access Hospital operating as a 43-bed facility offering acute beds, swing beds, and a subacute unit. The facility provides interdisciplinary support through social services and is part of a larger health system with multiple nearby sister facilities.
