Opportunity:

CONTRACT - RN - CASE MANAGEMENT (846) - 13 WEEKS - 8:00A-4:30P

Job Details:

* Day shift position working 8:00 AM to 4:30 PM
* 5 eight-hour shifts per week
* Rotating weekend schedule required
* No on-call requirements
* 13-week contract assignment
* Patient ratio of 25:1 for Case Managers and 40:1 for Social Workers
* Minimum 2 years of acute care Case Management experience required for RN Case Manager role
* Minimum 1-2 years of acute Social Work experience required for Social Worker role
* MSW preferred
* Provide case management services for both inpatient and observation patients
* Perform comprehensive assessments of psychosocial, medical, and discharge needs of patients and families
* Coordinate plan of care and drive discharge planning by collaborating with multidisciplinary team
* Evaluate progression of care using InterQual throughout episode of care
* Work collaboratively with RN case managers and multidisciplinary team to develop appropriate discharge plans for high-risk patients
* Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals
* Utilize equipment and systems including MIDAS, CAREPORT, INTERQUAL, and MEDITECH EMR
* Business casual dress code required
* Ideal candidate is calm under pressure, communicates clearly, builds trust, multitasks effectively, and works well as part of a team

Facility:

This facility operates a well-established Case Management unit located on the 4th floor of a medical plaza building. The unit is staffed by a team of 22, including Day Shift Case Managers, Social Workers, and Case Management Assistants. The department has demonstrated consistent performance excellence, maintaining a length of stay ratio recognized by administrative and division leadership for 10 consecutive months. Permanent positions are also available for those seeking long-term opportunities within the unit.
