Estimated Gross Weekly Pay: $2799.80

Care Manager

Job Details:

* Scheduling type is variable with weekend coverage required every other weekend, Saturday and Sunday, with a compensatory day off during the weekday
* Day shift patient ratio of 1:25 with a case load of up to 25 patients
* Patients served include adults and geriatric populations
* Business attire or scrubs required
* Epic charting system experience required
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, coordinating care, and discharge planning required
* Computerized documentation experience required
* Knowledge and experience with CMS, CPT coding and billing, DRG, HIPAA guidelines, and Department of Health regulations required
* Skills required include: Acute Hospital, Admission Criteria, Care Coordination, Determine Medical Necessity per Evidence-Based Guidelines, Disease Management, ICU, MS, Needs Assessment/Order DME, Plan of Care, and SDU/PCU/IMC/Obs
* California state license required and must be active upon start date
* Pending license accepted but must be obtained before start
* No floating expectation
* RTO restricted to no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Orientation hours: 40 hours
* Travel experience required; first-time travelers will be screened

The facility utilizes Epic as its electronic charting system and supports a multidisciplinary care environment focused on care coordination, discharge planning, and disease management.
