Estimated Gross Weekly Pay: $3464.40

Opportunity:

RN - Case Management/Care Coordination

Job Details:

* Work 8-hour day shifts with hours between 8:00 AM and 5:00 PM
* Every other weekend required, with at least one full weekend per month
* Holiday shifts not required but available if willing
* First to float as needed
* See 15-20 patients/consults per day
* Cover all areas of the hospital, excluding outpatient and home care
* Complete initial assessments, discharge planning, and utilization review
* Work with all payers including uninsured and homeless populations
* Utilize Interqual criteria for utilization review (training provided)
* Document in Cerner charting system
* Business casual attire required
* BSN required
* Must be familiar with Interqual EMR
* Key skills required: collaboration, teamwork, effective communication, and ability to reprioritize throughout the day
* Required certifications: BLS
* 2 supervisory references required at submittal (charge nurse accepted); 1 within 12 months and 1 within 3 years
* Must have prior recent CA Dignity experience to be considered

Facility:

This acute care facility is a Level 2 Trauma Center offering comprehensive inpatient services across all areas of the hospital. The facility provides 24/7 respiratory therapy and hospitalist or intensivist coverage, supporting a collaborative care environment. The case management department focuses on both discharge planning and utilization management, serving a diverse patient population including all payer types.
