Opportunity:

Case Manager

Job Details:

* Minimum 2 years of acute hospital case management experience required
* Travel experience required
* No first-time travelers accepted
* Patient ratio of 1:35, with a caseload of up to 25 patients daily
* Required to monitor up to 2 units on weekends with a focus on discharge planning
* Every other weekend required with mandatory holiday coverage
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restricted to no more than 7 days
* Scheduling type is variable
* Dress code: Scrubs or business casual
* Computerized documentation required; Epic charting system experience required
* MSW certification required
* Compact license must be in hand at time of submission
* Serve patient age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Perform both care coordination and discharge planning roles
* Conduct biopsychosocial assessments and participate in treatment plan development
* Knowledge of drug/alcohol abuse treatment options required
* Experience with patients experiencing homelessness, mental health history, or in crisis required
* Knowledge of POLST, AIDA, and PASRR required
* Ability to organize and participate in family meetings
* Coordinate services for home care and equipment following discharge
* Work with multidisciplinary team to ensure continuity of care
* Travelers may be expected to float between nearby sister facilities; refusal to float may result in termination or forfeiture of guaranteed hours
