Job Details:

* 12-hour shifts from 7:00 AM to 7:00 PM
* Every other weekend requirement
* Every other major holiday requirement
* No on-call or standby requirements
* No floating requirements
* 2-3 shifts of unit/department specific orientation
* Nurse acts as charge nurse, passes medications, and performs wound care
* Patient ratio of 1:10 on day shifts and 1:19 on night shifts
* Common diagnoses include heart disease, diabetes, and dementia
* No IV or titratable drips required
* No telemetry monitoring required
* Oxygen therapy equipment includes concentrators and portable concentrators
* Long-term care experience required
* No blocked schedule and no self-scheduling
* Scrubs must be clean and presentable, no specific color requirement
* Required certifications: BLS
* BSN not required
* EMR system: MatrixCare/Matrix

This long-term care facility operates a 26-bed unit providing specialized care for residents with chronic conditions such as heart disease, diabetes, and dementia. The facility offers a post-acute care setting with an interdisciplinary support team including physical therapy as needed and a social worker designee. Physician support is available by phone after hours, and the facility maintains a focused long-term care environment without rehabilitation beds.
