Care Manager

Job Details:

* Scheduling type is other with weekend coverage required every other weekend, both Saturday and Sunday, with a day off during the weekday
* Day shift patient ratio of 1:25 with a case load of up to 25 patients
* Patients include adults and geriatrics
* No floating expectation
* Business attire or scrubs required
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, coordinating care, and discharge planning required
* Computerized documentation experience required
* Epic charting system experience required
* Experience with acute hospital settings, admission criteria, CMS, CPT coding and billing, Department of Health regulations, determining medical necessity per evidence-based guidelines, disease management, DRG, HIPAA guidelines, ICU, MS, needs assessment and ordering DME, plan of care, and SDU/PCU/IMC/Obs required
* California state license required; pending license accepted but must be licensed upon start date
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restrictions of no more than 7 days; all RTO requests must be submitted upfront during submittal
* 40 orientation hours required
* Travel experience required; first-time travelers accepted but must be screened

This is an acute care facility offering a wide range of medical services to adults and geriatric patients. The facility supports interdisciplinary care coordination and discharge planning within a structured and professional environment. The organization maintains high standards for patient care and utilizes Epic as its primary charting system to support clinical documentation and care management.
