Opportunity:

Care Manager

Job Details:

* Variable 5-day workweek with department operations occurring 7 days per week
* Days off will vary and are not guaranteed to be back-to-back, although every effort will be made to schedule consecutive days off when possible
* Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* Patient ratio of 1:25 with a daily census of up to 25 patients
* Patients served include adults and geriatrics
* Float to the Laguna Facility may be required
* Minimum 2 years of acute medical Care Manager experience performing Care Coordination required
* Travel experience required - no first-time travelers accepted
* California state license required; pending license accepted but must be licensed upon start date
* Epic charting system experience required - hard stop requirement
* Scrub color: Turquoise or Business Casual
* Experience with assessing patients, care coordination, and discharge planning required
* Ability to organize and participate in family meetings
* Participate in the development of treatment plans
* Experience with HMOs, IPAs, etc. required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directives, POLST, and protective services process required
* Must be competent with arranging DME, SNF, and ambulance transport independently
* Computerized documentation required
* Skills required include: Acute Hospital, Appeals and Denials, Care Coordination, CMS, Department of Health, Disability Case Management, Discharge Planning, Disease Management, DRG, Emergency Department, HIPAA Guidelines, ICU, Needs Assessment/Order DME, OSHA, Plan of Care, Surgical Services, The Joint Commission/Core Measure/National Safety Goals, Worker's Compensation Case Management
* Trauma Level I and Level II experience preferred
* RTO restricted to no more than 7 days

This is an acute care facility that provides comprehensive medical services to adult and geriatric patient populations. The facility supports interdisciplinary care coordination and utilizes Epic as its primary charting system.
