Estimated Gross Weekly Pay: $2631.80

Opportunity:

Social Worker

Job Details:

* Weekend shifts required on a rotating basis to meet department needs
* Float required within the hospital only
* Average daily census of 40-50 patients
* Serve patient age groups including neonates, adolescents, infants, adults, pediatrics, and geriatrics
* Minimum 2 years of acute medical hospital social work experience required
* Travel experience required
* Assess patients, coordinate care, and manage discharge planning on a consult-based model
* Knowledge of drug/alcohol abuse treatment options, homelessness resources, mental health crisis intervention, advance directives, POLST, protective services, and adoption/foster care required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with Epic charting system required
* Trauma Level I experience preferred
* Business casual dress code required
* Required certifications: LCSW, MSW
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* May be expected to float between nearby sister facilities
* RTO requests limited to no more than 7 days and must be submitted upfront during submittal
